Many NHS staff do not feel safe enough to "Speak Out" and challenge colleagues over issues including patient safety, bullying and misconduct. There is a fear that "Speaking Up" carries consequences including rejection, being judged by others and career derailment. As human beings, we are hardwired for connection and belonging, and challenging the status quo can threaten our access to that. As Sarah, a Matron for Community Paediatrics, says,
